puruṣottame prabhu-pāśe bhagavān ācārya
parama vaiṣṇava teṅho supaṇḍita ārya

Translation: 
At Jagannātha Purī, in the association of Śrī Caitanya Mahāprabhu, lived Bhagavān Ācārya, who was certainly a gentleman, a learned scholar and a great devotee.
